Older Adult Services
The mission of the Older Adult Services at Jewish Family Service is to promote
and preserve the dignity, self-respect and independence of the individual to the fullest extent possible.
Senior Service Staff, Clinical Social Workers and experienced, compassionate volunteers work together to
coordinate services for older adults living in all residential settings. Jewish Family Service seeks to
assist elderly and disabled citizens by offering them a broad range of services including informational,
referral, and advocacy services.

Case Management Program
The case management program provides skilled and coordinated case management services and volunteer
assistance to senior and/or disabled adults to help these individuals remain independent in the least
restrictive environment for as long as possible. The program also provides skilled case management services
and advocacy to residents of long term care facilities.
A JFS Older Adult Services Professional evaluates the individual’s living situation and needs. After
consultation with the individual and/or family, a personalized care plan is developed to best meet the
physical, mental social and emotional needs of the individual. Senior Service Staff will coordinate,
implement and monitor his/her personalized care plan.
Counseling Services
Jewish Family Service provides individual and family support to help cope with adjustment problems related
to the aging process and assistance in long term care planning. Counseling is provided by Licensed Clinical
Social Workers.
Kosher Meals on Wheels Program
The goal of the Edward Teddy Cohen Kosher Meals on Wheels Program is to enable homebound elderly individuals
who cannot food shop for themselves to remain in their own homes for as long as possible by providing them
with well balanced, nutritious meals that meet or exceed the minimum daily requirements for health living.
Two’s Company Program
The purpose of the Moses “Buzz” Rosenberg Two’s Company Program is to enable homebound older adults and
disabled individuals to maintain their independence and reside safely in their own homes and out of
institutions for as long as possible through the intervention of trained JFS volunteers. Through the Two’s
Company Program, volunteers are matched with clients to provide daily life services, such as food shopping,
friendly visits, reassurance phone calls, occasional transportation to medical appointments and monitoring
the home for safety issues.
The Breakfast Club is a discussion group that gathers once a week at Jewish Family Service to provide
socialization, stimulation and interaction among homebound clients.
Power of Attorney and Guardianship Services
Jewish Family Service has years of experience serving as POA and Legal Guardian for members of the community.
As designated POA or Guardian, JFS staff provide ongoing case management services to ensure the safety and
well-being of their clients, collaborate with health-care professionals, attend care conferences, handle
all financial matters, and maintain ongoing personal contact and interaction with all their clients.
Financial Management/Billing Services
An Older Adult Services Professional will assist the individual with bill paying, checkbook balancing, and
proper completion of insurance and entitlement forms.
Educational and Support Groups
The Senior Service Department offers groups for caregivers that are feeling
overwhelmed and “burnt out”. Topics include: how to take better care of yourself
and your loved ones, community resources, alternative living arrangements
and long term planning for individuals and families.
